How can I accurately measure rake using simple tools such as a level and/or tape measure?

So many measure center of hub to fenders, but that is not accurate.
First make sure that you have your tire pressure the same, in all tires. Next, make sure that you are on a fairly level driveway, or garage cement floor. Then measure from either your rock rails, or pinch seam, at the rear, just a head of the rear wheel. Next take the measurement at the front, just behind the front wheel. The difference, will tell one what your rake is.

Is that measuring from the rock rail to fender flare bottoms at both front and rear points?
Floor (driveway, or garage) to rock rail, at both front and rear points. The vertical measurement from the ground to the rail.
