Need some help with FCA/Tread Lightly control number entry.

Need some help with FCA/Tread Lightly control number entry. My dealer is trying to validate the control number and it does not seem to work. He has called them up and it is there and valid. The problem is likely the ‘Sponser’s Last Name’. What should go here? He has tried all kinds of stuff and it is still not found. Suggestions?

Just pulled mine up and the 9th character of mine looks like it's the letter O, any chance that's being entered as a zero?
