I didn’t need an extender, but my spare is only 34” (LT285/75R17). I also requested the reverse brackets when I ordered my 1Up, which angle the rack down instead of up.
Just purchased a 10’ SAE socket and wiring harness and ran it to the driver’s side of my bumper. That’s about it. If you were to go to the passenger side, a 5’ harness would be fine. I use a Noco Genius5 battery tender so I also needed an adapter.

One 2x4 is enough and you really only need about 3’ of 2x6. I cut out a square of carpet on top of the dolly, sat the 2x4 on top and attached it with a few screws from underneath. Hope that makes sense.

If possible, find a dolly with better casters. these are fine for me because my doors stay off from April to November typically, so I'm not moving them around a lot. If you take your doors off more often, better casters would be an improvement.
